FAQ

Do you take Insurance

No, FIT LIFE THERAPIES does not take Insurance, and is considered "Out-of-Network" with all insurance companies. Payment will be due at the time of service. You will receive all necessary documentation to submit a claim with your insurance company, pending your out-of-network benefits, for possible reimbursement. 

How do I know if I have Out-of-Network Benefits

Contact your health insurance company, below are common questions to ask regarding your physical therapy benefits:

  • Do I have an out-of-network deductible? If yes, how much?
  • How much has already been met?
  • What percentage of reimbursement do I have?
  • Does the rate of reimbursement change because I am seeing a non-preferred provider?
  • Does my policy require a written prescription from my primary care physician?
  • If yes, will a written prescription from any MD/DO or a specialist your PCP referred you to be accepted?
  • Does my policy require pre-authorization or a referral on file for outpatient physical therapy services?
  • If yes, do they have one on file?
  • Is there a $ or visit limit per year? If yes, what is it?
  • Do you require a special form to be filled out to submit a claim?  If yes, how do I obtain it? 

How do I schedule

You can schedule by calling, e-mailing or texting. You can also visit our "packages and pricing" page to book! 

What do I need to wear/bring to my appointments

  • You will need to complete in Intake form/Consent to treat form prior to being seen: this can be filled out via out website under "Forms", or e-mailed directly to you.
  • Nothing else is required other than wearing comfortable clothing that you're able to move around in.
